Sunday Family Registration

This registration is for families who have never attended Creekside and are interested in checking their children into Creekside Kids on Sunday.  If you have kids in the youth group, please feel to register them at the same time.  Although we don't have youth programming on Sunday mornings, this will speed up their registration process when they first attend. The first part is for the primary adult of this household. Your phone number allows you to check kids in. Then please add any other adults in the household and their phone numbers (so they can check kids in) and the kids.
